Teacher/Volunteer Biographical Info
Each August we give all parents and students a Student's Ministry
Survival Guide. It contains our purpose for ministry, information on the
year's events, and information on you as leaders. The information you provide
will help us to better partner with parents, and better minister to the students
over the next year.
Please complete this entire form and return to Renee Sleeper as soon as possible. Some of the information will also be used in the Student Ministry Newsletters to help the students get to know you a little better.
We will also need a photograph of you for the guide and for the newsletters.
You can send a digital photo of yourself to or leave a photo
in the Student Ministry mailbox.
